Q:What kind of qualifications are needed to apply for student group insurance?
A:Students of our school who have paid the insurance premium are eligible for application.
Q:Under what circumstances can apply for Student Group Insurance?
A:Those who have suffered accidental injury resulting in death, disability, hospitalization or outpatient treatment.
Accidental accidents refer to unexpected accidents not caused by disease.
Q:What documents are required?
A:Please bring with the following documents to the Health Services Division (1F, Holistic Education Village) to fill out the application form.
Original medical diagnosis certificate (photocopy needs to be stamped by the hospital).
Original medical receipt (photocopy needs to be stamped by the hospital).
X-ray film are required for fractures.
A copy of the cover book of the passbook.
A copy of the student ID card or Certificate of Enrollment.
(Student under the age of 18 must have the signature of parent.)
Q:What is ‘Emergency aid grants by Education Property Foundation of Ministry of Education’?
A:The Ministry of Education uses the Education Property Foundation to offer condolences to students in emergency situations. (Exclude those studying in the master’s and doctoral classes of colleges and universities, or students over the age of 25.)
Q:Under what circumstances can apply for the “Emergency aid grants by Education Property Foundation of Ministry of Education “?
A:Can apply if one of the following conditions is met, but will not be approved if the total income in the latest year exceeds NTD$1 million, or the real estate value exceeds NTD$10 million, or intentional and illegal behavior of student.
Student has been hospitalized for injuries or illnesses for more than seven consecutive days or has died unexpectedly.
Student’s parent or guardian has been missing for more than six months, serve in prison, or involuntary separation.
Student or his/her parent or guardian meets the standard of catastrophic illness of National Health Insurance.
Death of student or his/her parent or guardian.
Student’s parent or guardian was injured and hospitalized due to special disasters.
Q:Is there a deadline to apply?
A:Please apply to the school within three months from the date of the fact.
