1. Log into your i-touch account or scan the QR code labeled Student Health Examination Form above to ensure that you have completed the Student Health Examination Form (CYCU) before March 10^th, 2025.
   
   

2. Please complete the health exam within the designated date; only cash payments will be accepted on-site after the health exam.

3. Remember not to have an empty stomach before the health examination. Also, avoid staying up later or engaging in intense exercise before the health exam to prevent any potential issues involving the accuracy of the report.

4. Please wear a mask throughout the entire process. If you feel unwell, are currently menstruating, or are pregnant, kindly inform the medical staff beforehand.
   
   
5. Starting from 2025/03/11, the medical examination will add an additional fee, the total cost for the examination will be 1000 NT$. Please complete the student health examination during 2025/02/01~2025/03/10 to avoid additional fees.
   
   
6. You may check your health examination report on Chi Hsin Clinic website after the examination for approximately 40 days .
   
   
7. Based on regulations from the Ministry of Education, all students in CYCU must cooperate and participate in the health exam, Refusing to comply with the regulation may cause increased risks or serious illness, and may be involved in improper consequences.
